Medical Claims Authorisation Forms 


From 30 June 2015, use these forms to make:

  • Withdrawals from your Medisave account;
  • Claims from MediShield Life; and/or
  • Claims from your Integrated Shield Plan (IP) for medical expenses.

Medical Claims Authorisation Form (Single Institution) – MCAF(S)

What is MCAF(S) and how will it benefit me?

This new claim form allows the medical institution to use your Medisave, MediShield Life and IP to pay for your medical treatment. It replaces two forms:

·         Medisave Authorisation Form (MAF)
·         Claim Form for Medisave-Approved Integrated Shield Plan (IP)

It is more convenient as you only need to complete and sign one form.

When will I need to use MCAF(S)?

You will need to sign the MCAF(S) if you want the medical institution to help you with any of the following:

  • Check how much Medisave you can withdraw for the treatment, and whether you can claim from MediShield Life or your IP
  • Withdraw from your Medisave to pay for approved treatments
  • Claim from your MediShield Life or IP to pay for approved treatments
  • Withdraw from your Medisave to pay for your immediate family member’s (parent, spouse, children, and grandparents) approved treatments

Where will MCAF(S) be used?

MCAF(S) will be used at all Medisave-accredited medical institutions. These include public and private hospitals, polyclinics, and Medisave-accredited General Practitioner (GP) and dental clinics.

Medical Claims Authorisation Form (Multiple Institutions) – MCAF(M)

What is MCAF(M) and how will it benefit me?

MCAF(M) is a new claim form for you to authorise payment for your current and future medical treatments using Medisave, MediShield Life and Integrated Shield Plan (IP) across all participating institutions.

Previously, you would need to sign a form at each medical institution you visit, each time you want to use your Medisave and MediShield. Now, you can choose to sign the MCAF(M) to cover all approved uses at all participating institutions, including new uses that are introduced and new medical institutions that join in the future.

When will I need to use MCAF(M)?

It is not compulsory to use the MCAF(M). It is provided as a convenience to you.

Where is the MCAF(M) valid?

The MCAF(M) is valid at all public healthcare institutions, such as public hospitals and polyclinics. Refer below for the latest list.

  • Your immediate family member can fill in the MCAF(S) form as an “additional Medisave payer” to authorise using his/her Medisave for your approved medical treatment.

  • Staff at the medical institutions will help you claim and withdraw from your MediShield Life/ Integrated Shield Plan (IP) and Medisave to reduce your cash payment.

    If you have a specific payment arrangement or do not wish to use Medisave for a particular visit, please inform the staff at the medical institution. You do not need to specify these arrangements in the form when you sign.

    Examples of specific payment arrangements include:
    ·         Using your family members’ Medisave;
    ·         Specifying how much Medisave you and your loved ones would like to use; and
    ·         Not wanting to use Medisave / MediShield Life / IP for a particular visit.

  • You will not need to. Your doctor and medical institution will provide CPFB and your insurer with the necessary information. They will contact you if they need further information.

  • Your information will be used to facilitate and process your withdrawals and claims, helping to make the process smoother for you. CPF Board and your insurer will obtain the necessary information from your doctor and medical institution after you have given your authorisation.

    As Medisave and MediShield Life are Government-run schemes, the Government and CPF Board may also use the information anonymously to help develop and review policies.

  • The Central Provident Fund (CFP) Board conducts audits to ensure that medical institutions do not submit unauthorised claims. Those who do so are subject to penalties, and have to refund your Medisave with interest.

    To protect yourself, you can check your CPF Medisave statements and notification letters which are sent to you whenever a claim is processed. Please alert the CPF Board if you find any wrong or unexpected deductions.

  • Authorisation for an inpatient episode is valid only for that particular episode.

    For authorisation for outpatient treatments, you can choose the length of authorisation, from a single treatment to lifelong.
