The Ministry of Health will be conducting the National Health Survey 2004 between September and December 2004. Invitation letters have been sent to 7,500 Singaporeans to participate in the National Health Survey 2004. They were randomly selected from about 18,000 individuals identified in some 7,000 household units in an enumeration exercise conducted between May and July 2004.
These 7,500 persons form a nationally representative sample of Singaporeans aged 18 to 74 years. Each of them has been requested to go to one of the following six polyclinics between September and December:
Yishun Polyclinic
Hougang Polyclinic
Ang Mo Kio Polyclinic
Outram Polyclinic
Tampines Polyclinic
Bukit Batok Polyclinic.
These polyclinics have been chosen as survey sites in order to provide easy accessibility for the selected participants. The National Healthcare Group Polyclinics staff will conduct the survey at these sites for the Ministry.
During the survey, the participants will undergo a free health screening worth $150 and answer questionnaires on their lifestyle practices and dietary patterns. The health screening will involve tests for diabetes, blood lipids and urinary proteins; and measurement of blood pressure, and height and weight.
All information on individual participants obtained from the survey will be kept strictly confidential. Any information that is obtained from the survey will only be used collectively to assess the health status of Singaporeans in relation to major problems such as diabetes mellitus, high blood pressure, high blood cholesterol, smoking and obesity. The results of the survey will also be used to evaluate the effectiveness of the government's national disease control plans, and to chart the progress towards achieving its long-term health targets.
The Ministry would like to encourage all Singaporeans who have been selected for the survey to give their full support and co-operation by participating in the survey. The Ministry would also like to seek the support of employers whose employees have been selected for the survey, to allow them to participate in the survey.
