ONE-TIME 1,500 HEALTHIER SG HEALTHPOINTS BENEFIT TO ENCOURAGE REGULAR HEALTH PLAN CHECK-INS

From 1 July 2026 to 31 December 2029, Healthier SG enrollees will receive a one-time 1,500 Healthpoints benefit worth $10 when they complete an in-person Healthier SG Health Plan check-in and view their updated Health Plan on HealthHub. This initiative supports Healthier SG’s focus on preventive care by encouraging enrollees to engage their Healthier SG doctors regularly, detect and manage any health conditions early, and develop health goals jointly.
2. Enrollees will need to complete the following to qualify for the Healthpoints benefit:
a. Complete an in-person annual Health Plan check-in in the year(s) following their first Health Plan consultation; and
b. View their updated Health Plan on HealthHub after completing an in-person Health Plan check-in.
3. These should be completed within the qualifying period of 1 July 2026 to 31 December 2029. After both actions are completed, the one-time 1,500 Healthpoints benefit (worth $10) will be credited to the enrollees’ Healthy 365 app account. Enrollees are encouraged to return for their annual Health Plan check-in only when they are due for one. The annual Health Plan check-in may be conducted during visits for other purposes, such as routine chronic appointments (prevailing fees for services outside the Healthier SG scope will apply).
Annual Health Plan Check-ins
4. Launched in July 2023, Healthier SG is our national initiative that focuses on preventive care and supports residents aged 40 and above to take action for better health. As of May 2026, over 1.4 million residents have enrolled in Healthier SG, which is 59% of the eligible population. Among these enrollees, 83% have completed their first Health Plan. 66% of the enrollees due for an annual Health Plan check-in in 2025 completed their check-in in 2025. There are currently more than 1,100 GP clinics and polyclinics participating in Healthier SG.
5. Under Healthier SG, residents can enrol with their preferred Healthier SG clinic and attend a fully subsidised first Health Plan consultation to create their personalised Health Plan with their Healthier SG doctor. This Health Plan would include health goals and recommended actions such as health screenings, vaccinations, and lifestyle modifications.
6. Subsequently, Healthier SG enrollees are encouraged to attend annual Health Plan check-ins with their Healthier SG doctor. These check-ins are a key step in the Healthier SG journey for all enrollees and are fully subsidised.
7. Annual Health Plan check-ins can be conducted in-person or via tele-consult. The recommended frequency of an in-person check-in depends on whether they have any chronic conditions. Regular in-person Health Plan check-ins allow Healthier SG doctors to assess enrollees’ health in-person and provide recommended screenings and vaccinations during the same visit where appropriate.
Artificial Intelligence (AI)-powered Exercise Planner in Health Plan AI (Beta)
8. To better support enrollees in achieving their health goals, a new Health Plan AI beta feature will be available on HealthHub from 27 July 2026 to 27 Jan 2027. Health Plan AI aims to complement Health Plan discussions that enrollees have with their Healthier SG doctors. It will offer personalised exercise plans with suggested options to achieve the exercise goals set in the Health Plan, based on demographic factors and individual preferences.
9. During the beta launch, Health Plan AI will be available to enrollees between 40 to 64 years old with no chronic conditions, and have an existing Health Plan. Eligible enrollees can access Health Plan AI through the Healthier SG module on HealthHub. MOH will review the outcomes of the beta before deciding whether to make it a permanent feature.
