Question No. 2304
To ask the Coordinating Minister for Social Policies and Minister for Health (a) in each year since 2023, what proportion of Healthier SG enrollees discharged from (i) public hospital wards and (ii) public specialist outpatient clinics to primary care have been referred back to their enrolled clinic, compared to any other clinic; and (b) whether the Ministry will set this proportion as an indicator for the healthcare clusters.
Answer
1 Public healthcare clusters will try to discharge their patients back to their Healthier SG enrolled clinics. In 2025 and 2026, about 75% of National Healthcare Group (NHG) Health patients from specialist outpatient clinics were discharged to their enrolled clinics. Statistics are not currently available for other healthcare clusters and they are progressively putting in place the processes to track this as an indicator.
