REVOCATION OF LC NURSING HOME (PTE.) LIMITED’S LICENCE TO PROVIDE NURSING HOME SERVICES

The Ministry of Health (MOH) has issued a notice of licence revocation to LC Nursing Home (Pte.) Limited (LC Nursing Home) on 29 June 2026. In the notice, MOH has informed LC Nursing Home of our decision to revoke its licence to provide nursing home services with effect from 23 November 2026.
2 The licence revocation date takes into consideration the time required to transfer the 78 current residents of LC Nursing Home to other nursing homes. However, to safeguard the safety and well-being of nursing home residents given findings of serious and systemic lapses in compliance with requirements under the Healthcare Services Act 2020 (HCSA), MOH has deployed an interim care team to LC Nursing Home with effect from 29 June 2026 to ensure that residents continue to receive proper care throughout this process.
3 LC Nursing Home currently holds a licence under the HCSA to operate a 93-bed nursing home service. With the revocation of its licence with effect from 23 November 2026, LC Nursing Home will not be able to provide nursing home services at its permanent premises located at 2 Jalan Ulu Siglap, Singapore 457121.
Findings from MOH’s audit of LC Nursing Home
4 Following audits in November and December 2025, MOH had informed LC Nursing Home of findings of serious and systemic non-compliance with HCSA requirements. LC Nursing Home was then given the opportunity to remediate these non-compliances, with MOH enforcing stricter and closer monitoring before assessing if further regulatory action was necessary.
5 A further audit of LC Nursing Home was conducted in April 2026 to determine the effectiveness and sustainability of their rectification efforts. The audit found that LC Nursing Home had failed to fully implement certain rectifications or sustain rectification of earlier identified lapses, with new and repeated non-compliances with HCSA requirements. Lapses found include:
(a) Inadequate clinical and nursing care
(i) Failure to conduct appropriate reviews or monitoring for residents in multiple aspects including falls, restraint practices and pressure injuries;
(ii) Failure to adhere to wound care protocols; and
(iii) Poor medication management such as presence of poorly maintained medication stock, medication errors, and administration of medication by care staff without suitable credentials.
(b) Failure to provide adequate basic care to residents
(i) Failure to adhere to food hygiene practices and implement safe food processes with regard to meal preparation for residents.
(c) Inadequate infection prevention and control practices
(i) Failure to implement infection prevention and control measures; and
(ii) Failure to ensure environmental hygiene, and suboptimal pest control.
(d) Failure to ensure a safe environment
(i) Poorly maintained infrastructure and environment, with multiple hazards posing injury risks to both residents and staff.
6 MOH had on 2 June 2026 issued a notice to LC Nursing Home regarding the intended revocation of its licence. Key non-compliances were set out in the notice and LC Nursing Home was given the opportunity to submit representations within 14 days in respect of the intended revocation for MOH’s consideration. MOH received these representations on 16 June 2026.
7 In these written representations, LC Nursing Home did not provide any explanations to refute the non-compliances, but instead acknowledged MOH’s findings. While LC Nursing Home assured MOH that they would take remedial actions to address the findings, the plan provided by LC Nursing home was very brief, without clear milestones set, and did not provide sufficient assurance.
8 After careful consideration of LC Nursing Home’s representations, and the findings of MOH’s audit in April 2026, MOH assessed that LC Nursing Home is unable to continue providing nursing home services safely.
Ensuring continuity of care for LC Nursing Home residents
9 The interim care team from Vanguard Healthcare Pte Ltd deployed to LC Nursing Home from 29 June 2026 will ensure residents are properly cared for, before the residents are transferred to other nursing homes. MOH will continue to work closely with LC Nursing Home and Vanguard Healthcare Pte Ltd to monitor and ensure the safe and smooth transfer of the residents to other nursing homes.
10 Vanguard Healthcare Pte Ltd and the Agency for Integrated Care (AIC) will inform all affected residents and their next-of-kin of the revocation of LC Nursing Home’s licence, and prioritise arrangements for the residents to be transferred to other nursing homes.
MOH’s audit of nursing homes
11 The audit of LC Nursing Home was part of MOH’s audit of selected nursing homes with identified areas of improvement to gauge compliance with infection and prevention control practices, and basic custodial and nursing care. MOH will not hesitate to take regulatory actions if licensees and/or key appointment holders are found to have fallen short of regulatory requirements to deliver safe, adequate and appropriate care. The regulatory action taken on LC Nursing Home follows similar action taken against Windsor Convalescent Home Pte Ltd (Windsor Convalescent Home), after an MOH audit found serious and systemic lapses in Windsor Convalescent Home’s compliance with HCSA. Windsor Convalescent Home’s licence to provide nursing home services will be revoked with effect from 30 October 2026.
12 MOH takes a serious view of the lapses found in these two nursing homes. The decision to revoke both nursing homes’ licences was taken only after careful assessment that they could not provide adequate nursing home services, and that resident well-being and safety had to be prioritised and safeguarded. MOH will work closely with Vanguard Healthcare Pte Ltd to ensure no new residents are admitted in the interim period before the respective licence revocation dates of Windsor Convalescent Home and LC Nursing Home.
13 In addition, MOH intends to share the findings of its audit with the sector at a suitable platform and work with AIC to enhance support available for the sector. This includes training to equip the sector with the necessary skillsets to adhere to the required standards of care.
14 Licensees are reminded to comply with the HCSA, the applicable regulations, and conditions of their licences at all times. MOH will continue to monitor all nursing homes to ensure compliance with HCSA requirements.
