REVOCATION OF WINDSOR CONVALESCENT HOME PTE LTD’S LICENCE TO PROVIDE NURSING HOME SERVICES

The Ministry of Health (MOH) has issued a notice of licence revocation to Windsor Convalescent Home Pte Ltd (Windsor Convalescent Home) on 18 June 2026. In the notice, MOH has informed Windsor Convalescent Home of our intention to revoke its licence to provide nursing home services with effect from 30 October 2026.
2 The licence revocation date of four months from now takes into consideration the time required to transfer current residents of Windsor Convalescent Home to other nursing homes. However, to safeguard the safety and well-being of nursing home residents given findings of serious and systemic lapses in compliance with requirements under the Healthcare Services Act 2020 (HCSA), MOH has deployed an interim care team to Windsor Convalescent Home with effect from 18 June 2026 to ensure that residents continue to receive proper care throughout this process.
3 Windsor Convalescent Home currently holds a licence under the HCSA to operate a 45-bed nursing home service. With the revocation of its licence with effect from 30 October 2026, Windsor Convalescent Home will not be able to provide nursing home services at its permanent premises located at 369 Pasir Panjang Road, Singapore 118706.
Findings from MOH’s audit of Windsor Convalescent Home
4 An MOH audit in April 2026 found serious and systemic lapses in resident safety, clinical and nursing care, and infection control practices, compounded by a lack of control, governance and oversight by Windsor Convalescent Home’s key office holders. The lapses include:
(a) Inadequate clinical and nursing care
(i) Failure to conduct appropriate reviews for residents in multiple aspects including falls, pressure injuries and weight loss;
(ii) Failure to follow up on or adhere to the residents’ care plans; and
(iii) Poor medication management such as omission of medications, use of expired medications, and discrepancies in medication quantity.
(b) Failure to provide adequate basic care to residents
(i) Neglecting residents’ fundamental care needs, such as basic grooming;
(ii) Failure to provide appropriate nutrition for residents based on their individual needs; and
(iii) Failure to store food ingredients used to prepare meals for residents appropriately, with some ingredients found to be expired.
(c) Inadequate infection prevention and control practices
(i) Failure to implement infection prevention and control measures; and
(ii) Failure to ensure hygiene and sanitation in housekeeping processes.
(d) Lack of governance and oversight by key office holders
(i) Failure of the key office holders to exercise clinical governance over the care delivered to residents; and
(ii) Failure to provide oversight of the operation and maintenance of the premises.
5 MOH had on 5 May 2026 issued a notice to Windsor Convalescent Home regarding the intended revocation of its licence. Windsor Convalescent Home was given the opportunity to submit representations within 14 days in respect of the intended revocation for MOH’s consideration. MOH subsequently extended the period for Windsor Convalescent Home to submit representations to 28 days, following Windsor Convalescent Home’s request for an extension. MOH received these representations on 3 June 2026.
6 After careful consideration of Windsor Convalescent Home’s representations, and the findings of MOH’s audit in April 2026, MOH assessed that Windsor Convalescent Home is unable to continue providing nursing home services safely.
Ensuring continuity of care for Windsor Convalescent Home residents
7 MOH has deployed an interim care team from Vanguard Healthcare Pte Ltd to Windsor Convalescent Home with effect from 18 June 2026 to ensure residents are properly cared for, before the residents are transferred to nursing homes of their choice and eligibility.
8 Vanguard Healthcare Pte Ltd will inform all affected residents and their next-of-kin of the revocation of Windsor Convalescent Home’s licence, and work with the Agency for Integrated Care (AIC) to make arrangements for the residents to be transferred to other nursing homes of their choice and eligibility. MOH will work closely with Vanguard Healthcare Pte Ltd to ensure no new residents are admitted in the interim period before the licence revocation date of Windsor Convalescent Home.
MOH’s audit of nursing homes
9 The audit of Windsor Convalescent Home was part of MOH’s ongoing audit of selected nursing homes with identified areas for improvement to gauge compliance with infection and prevention control practices, and basic custodial and nursing care. MOH will not hesitate to take regulatory actions if licensees and/or key appointment holders are found to have fallen short of regulatory requirements to deliver adequate and appropriate care.
10 In addition, MOH intends to share the findings with the sector at a suitable platform, and work with AIC to enhance support available for the sector. This includes training to equip them with the necessary skillsets to adhere to the required standards of care.
11 Licensees are reminded to comply with the HCSA, the applicable regulations, and conditions of their licences at all times. MOH will continue to monitor all nursing homes to ensure compliance with HCSA requirements.
