Transforming the primary care landscape: Engaging the GP community and our stakeholders in the journey (2011)

“GPs are well-positioned to play this important role in providing accessible, holistic and continuing care for our patients”.This was shared by Health Minister Gan Kim Yong shares at the Primary Care Workplan Seminar on 8 Oct. Our growing and ageing population, longer life expectancies, and changing lifestyles are contributing to a greater demand for chronic care. There is thus a need to better support GPs, who see about 80% of primary care attendances, so as to achieve better care outcomes.
