Community Health Assist Scheme

The Community Health Assist Scheme (CHAS) enables all Singapore Citizens, including Pioneer Generation and Merdeka Generation cardholders, to receive subsidies for medical and/or dental care at participating General Practitioner (GP) and dental clinics.

Who is eligible?

All Singaporeans, regardless of income, will be eligible for CHAS. The CHAS benefits will be tiered according to household monthly income and Annual Value (AV) of home, as indicated in the below table: 

CHAS types 2024 AV

^ Household monthly income per person is the total gross household monthly income divided by total number of family members living together. Gross monthly income refers to your basic employment income, trade/self-employed income, overtime pay, allowances, cash awards, commissions, and bonuses.

^^ The AV of your home is the estimated annual rent if it is rented out. It is assessed by IRAS. Existing CHAS cardholders who are eligible for a CHAS card with higher subsidies as a result of the revision of AV thresholds from 1 January 2024 will be automatically issued with new CHAS cards based on the residential property AV as of 31 December 2023. New cards will be made available from February 2024. No action is required from these cardholders.

Amount payable by patient = Total clinic charges – CHAS subsidies

CHAS benefits 2024 AV 

Capped at 24 visits for common illnesses per patient per calendar year, across all CHAS clinics, from 2020 onwards

 2 “Simple” refers to visits for a single chronic condition. “Complex” refers to visits for multiple chronic conditions, or a single chronic condition with complication(s).

 3 This refers to screening for diabetes, high blood pressure, high blood cholesterol, colorectal cancer and cervical cancer. For Healthier SG (HSG)-enrolled Singapore Citizens, nationally-recommended screenings under Screen for Life will be fully subsidised at their enrolled HSG clinic. You can check with your enrolled HSG clinic when your recommended screenings are due.

CHAS Dental Subsidies

Amount payable by patient = Total clinic charges – CHAS subsidies

CHAS Dental Subsidy Table (1 Nov) 

As a CHAS, Merdeka Generation (MG) and Pioneer Generation (PG) cardholder, you can also enjoy subsidised referrals to Specialist Outpatient Clinics (SOCs) located at Public Hospitals, National University Centre for Oral Health*, Singapore (NUCOHS), and National Dental Centre* when required.

*Only for CHAS Blue/Orange, MG and PG cardholders.

How can I apply?

You can apply for CHAS via the online application by using this link.

Alternatively, you can also pick up a CHAS application form at any Community Centre or Club (CC), Community Development Council (CDC), Public Hospital or Polyclinic or download the application form by using this link [PDF, 631KB]. Please send the completed and signed application form to P.O. Box 680, Bukit Merah Central Post Office, Singapore 911536.

If anyone in your household is not a Singapore Citizen, please also include copies of their foreign identification documents (e.g. work permit/long term visit pass).

How do I use the CHAS Scheme?

If your application is successful, all eligible members of your household will receive an individual CHAS card in their name. The card indicates the subsidy tier you qualify for (CHAS Blue or Orange or Green) and is usually valid for two years. Pioneer Generation and Merdeka Generation cardholders can use their Pioneer Generation or Merdeka Generation card at any CHAS clinic to obtain CHAS subsidies.

Simply present the card at a CHAS-participating GP or dental clinic when seeking treatment.

You will receive an itemised bill for your treatment clearly outlining the costs for treatment.

Points to note

First time visit: Please also present your NRIC upon registration (or birth certificate for cardholders under 15 years old).

By using your CHAS or Pioneer Generation or Merdeka Generation card at a CHAS clinic, you consent to allow your doctor/dentist to disclose your personal information to the Ministry of Health (including its authorised agents).

  • Mondays to Fridays from 8.30 am to 6.00 pm (excluding Public Holidays).
    On the eve of (i) Christmas, (ii) New Year and (iii) Chinese New Year, the CHAS hotline will operate from 8.30 am to 1 pm.
