MediSave for the Chronic Disease Management Programme (CDMP)

Chronic conditions are a significant cause of illness and death in Singapore, similar to other developed countries.

If not properly diagnosed and managed, these conditions often lead to more serious complications, which may result in the need for costly treatments and hospitalisations. However, with early detection and good management of these chronic conditions, complications can be avoided or delayed. Individuals are therefore encouraged to have a regular family doctor, undergo recommended health screening, and, if diagnosed with a chronic condition, actively manage their conditions through regular monitoring, appropriate and timely medical treatment, and lifestyle changes.

The Chronic Disease Management Programme (CDMP) was first introduced in Oct 2006. It now covers a total of 23 conditions:

  • Diabetes Mellitus / Pre-diabetes
  • Hypertension
  • Hyperlipidemia (Lipid Disorders)
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Chronic Kidney Disease (Nephritis / Nephrosis)
  • Schizophrenia
  • Major Depression
  • Bipolar Disorder
  • Anxiety
  • Stroke
  • Dementia
  • Osteoarthritis
  • Parkinson's Disease
  • Benign Prostatic Hyperplasia
  • Epilepsy
  • Osteoporosis
  • Psoriasis
  • Rheumatoid Arthritis
  • Ischaemic Heart Disease
  • Allergic Rhinitis
  • Gout
  • Chronic Hepatitis B

To ensure that this programme is accessible to all, the Ministry has engaged the support of more than 1,250 GP clinics around Singapore to provide systematic, evidence-based chronic disease management under the CDMP. The CDMP is also available at all polyclinics, all public hospitals’ Specialist Outpatient Clinics (SOCs) and participating private specialist clinics.

Under the MediSave 500/700 scheme, CDMP patients can use up to $700 per year if they have complex chronic conditions, or $500 per year otherwise, to reduce the out-of-pocket cash payments for outpatient chronic disease bills. Each claim is subject to a 15% co-payment in cash. Since 1 Jul 2014, the $30 deductible for CDMP claims has been removed. With the coverage of CDMP aligned with that under CHAS Chronic, eligible patients can tap on both their MediSave and CHAS Chronic subsidies for the outpatient treatments of their CDMP conditions at CHAS GP clinics.

Greater Support for CDMP Providers

Providers can submit patients’ MediSave claims and selected clinical indicators to MOH via web-based platforms such as Mediclaim Online and the MOH Healthcare Claims Portal (MHCP), and integrated clinic management IT solutions such as GPConnect and other Smart Clinic Management Systems (SmartCMS) which allow seamless information flow from the clinic’s CMS to the relevant claims system and clinical data repository
